They sent me the same email! They attached a form with my husband's application number and address. When my husband contacted the doctor'S office they asked to print one more form from CIC website. Call the doctor asap the know exactly what you need.. good luck!!!

I think this is a general email that they send to everybody, do not feel bad. They just want you to wait until they ask in formal for your passport, remember that you have to attached that letter when you send it. I have hope and I know my file is below yours
