Question lang po... Sa mga nagpaconsult sa cic nung previous fswp at ngayon ay diy nalang, finill-up nyo pa po ba ung "use of a representative" na form para icancel ung sa cic?

Yes, this is true. My forms were passed for fswp 2013 by cic, ths consultant. But i did not reach the cap.. I filled up the "use of a representative" form then but for fswp 2014 i am planning to do all this by myself. Do i still need to fill-up the form to CANCEL the use of a representative?
