There is another way to inform CHC for change in mail address of applicant. You can fax them the letter giving the details of change in address and also state the reason for this change.and give your full details i.e UCI number which u received in sponsorship approval , your name , DOB...

I think Applicant is "sponsored spouse in pakistan" and leave the spouse column blank. in dependent column ur child's info (if u have). Hope this would help u.
