Kummy29 & Mitali, I do not know about the case with other countries, but with NDVO... I am certainly sure what Harjot says is correct... They have stopped sending the forms now and the DMP provides them when we go there... So, Dr. Bhatia, nothing to worry... Just go with the Cover Letter and...

The inner lines don't matter much... they can go haywire at times... Just wait for DM now :)

Have to go to second floor at PC office... A month, I think, is a little extended time... Could take about a week, if you follow up well... Also need to take along original PPs and marriage certificate, if applicable... :)
