Thanks for responding. I talked to IRCC on the phone yesterday. I was told to select “apply for a work permit with the same employer”. Since you have an open work permit, your current employer is “open”, you are not looking to change this. So don’t select “first time or new employer”.

Awesome, good to hear this. I just applied for open work permit extension while waiting for my PR (it's been 6 months). My medical is expiring in 2 months so there is a possibility that my medical might expire while my open work permit or PR is processing.

What do you mean by medical extension? Did you have to get medical exam done again?

This is not true in this case: If you answer YES to "Have you had a medical exam performed by an IRCC authorized panel physician (doctor) within the last 12 months?" then you will be asked to provide the proof of upfront medical exam.
