45 days run for every thing except pcc. I had the same problem b4 and VO told me that 30 days for other papers that I have to send and pcc should be send once completed. So don't worry I had the same situation and I got my visa

depends on how many countries you have been visited. Also, if your criminal record is clear :) 80% of cases finished within 6 months. Those have problems in Background check got delayed. Wish you all the best
