What date did you do your medicals? Was it about 10 days before AOR?

Same here. For me they still say medicals not required. AOR March 30th. When did you get your medicals done? Mine were on March 28th.

May be you did not select the option of paying both the fees together. It asks for that confirmation before you start filling the ITA form I remember. Based on that it give you total fees to be paid after you upload the documents.

Hi DEEPCUR, Thanks. I have got it done today. I am attaching the documents now. Under what category this affidavit needs to be attached? I don't think I need to provide LOE for anything so that space is available or Should I attach it in educational certificates itself?

You might not need a sample because every bank has their own specific format. Just show them the requirement from the CIC website & they should do the needful. I got it done the same way.
