The data I have from June 2010 Applicants forum is that you have applied on June 17. Which date is correct?

I have submitted 6mo bank statement with the full docs. That money is in an FD but, I live in a different city now. I hope I don't have to visit the branch for the POF letter. One less document is one less headache. Goodluck with medicals and the visa!

I think you should inform them by email by attaching a copy of your daughter's passport and birth certificate. They will probably advise you on what to do. You can probably use their email as a supporting document to use the real name along with medical forms. Check in this forum on name...
