Hi there Edgehead. Cool. No they didn't request anything from my passport at all. Although, I did provide my passport photo page photocopy in my application (even though it wasn't requested) So maybe that helped a bit. Thanks for your message!

Thanks! This one was kinda weird that they would request it, I DID fill this one out in my initial application, mind you the one they sent me looks a little different. But, I'll do it again :)
