If I were you, I would not answer the question. I would say "See additional papers" and then submit everything including an explanation of exactly what happened.

I would go with option 2. Use your parents address and send an additional letter explaining your living circumstances.

I'm pretty sure you have to apply for a visitor visa extension. That's what I did. I am now allowed to stay here until May 2013 and my PPR will hopefully be here way before that.
