Well u can send multiple address of same city. Ideally all addresses after age of 18 years must be covered. If u have been in more than one city then u have to take PCC from all cities.

Yes they need bank statement. U need to send only bank statement. First please go through cic website thoroughly your 99% queries will answered there. Rest u can ask here in forum. Or just study Qorax thread from start

Normally people use it, u do need to keep some amount in that account. Never zero it. They will ask for bank statements two times at least during process

Go to DPO office, with 4 pictures, PP copy, NIC, they will give u form, fill it and submit it. They will verify it from your respective police stations of the addresses u mentioned. Normally they provide in 4,5 days.

In your CSE what file no you mentioned? B------- or E------- Please mention new file no starting with B most probably. If u have already mentioned Newfile no. Then contact IOM Islamabad and show them email print. Last be patient. Thanks
