No. Don't do it - specially the generic form. Give the current date where applicable. Also give your UCI. Yes man i filled the new generic form as i already said they had put a cross on it.

then what did you mean to say by writing "they have already removed the transcripts"? They have removed the transcripts from the sealed package. and punched it and sent it back….sorry for the confusion.

What do you mean by retaining of transcripts? what all document they didnt return? Have they sent any letter confirming that they are retaining some of your documents? they didn't retain anything.
