Did you notify them of any previous visa refusals when submitting your application? Some embassies like UK usually leave a note after refusal while some like US just tell you verbally. So you can explain with LOE.

But in the application guide, there was a clear instruction to tag passport as e.g 'John Smith Passport photo' when uploading. I did that and I did not write my name at the back of the photo. Now..I'm not sure if I did the right thing.
