This means the officer was finished with your file but realized he/she missed the fact that your medical was expired so for the sake of due diligence, asked for medical. Refugee PR application processing is so random these days, you can't predict what happens next.

It is a pre-written letter. When you go there, they will not do biometrics for under 14. Take a passport size photo just in case.

There is no requirement for children under 14 to do biometrics.
