The BLS checklist is a copy of the CGI checklist arranged in a document format. The instructions for both are same.

Also, a few things to add on: 1. No original documents were kept. 2. Take prints of the entire checklist. Including instructions. They will keep those too.

You can't apply for surrender and OCI at the same time. You need the surrender cert/cancelled passports back to apply for OCI, and that will be delivered to a Canadian address.
