Hi.. No my mothers given name did not change after marriage. I was worried about the family name given by birth. It will be LUHAR. And her given name will be MAKWANA BHARTI RAJESH. (As per passport) But if IRCC asks for her LUHAR family name documents i dont have any. Will that be ok.??

As per my knowledge It would be maiden's name for family name by birth. Because it says FAMILY NAME BY BIRTH. @legalfalcon please help.. I think he is occupied somewhere. Otherwise he would have definately solved our query

Did you àdd extra pages where the table space is not enough?? I have 10 addresses and just 5 columns, so is it ok to add the 6th to 10th address in extra sheet?? Plus in mothers given name at birth? It will be the given name before marriage right? @legalfalcon @ImpatientAlligator or anyone...

This is a good idea. This would help the agent understand better. So for eg if i have 10 addresses and just 5 columns then I need to attach extra sheet for 6th to 10th address. is it so?
