My son has just turned 17 and under my application too. I was told that they only get background complete Nothing to worry about regarding this. Once you're done with the oath...he will be too, but I think you have a bigger issue than that maybe based on what I read here. Good luck. Hopefully...

Yes Because you may have an issue You need to match your name the way you applied with. Especially, if that's how it's written in your passport

You will need to write your name in the application exactly the same way it's written on your PR and other Canadian IDs.

Call them and ask to send you the form. Not sure they do it via phone call
