    Visa offices were advise by NHQ finance (FSWfeereturn Office) to send such information to Pre Feb 2008 who proactively contacted them (see www.cic.gc.ca/notice of 12/10/2012) and if you see the reply from FSWfeereturn office after submitting the form it is stated below that CIC will only action...
