The DMP office asked for US Passport SIze Photographs. To be on safer side,i took Canada Passport size,Canada Visa,my Home country Passport and Visa size photos also.

USD 300 for each adult USD 150 for below 5 year Kid USD 200 for 5-11 Year Kid These are the rates for that particular DMP in New Jersey.Other DMP's in other states may have their own rates.
