    What I did was email all the provinces. Here is the webpage http://rcpsc.medical.org/links/provli_e.php. I emailed all of them, explained my particular situation, and asked them what i would need to do. Most of them responded timely and were very friendly. Hope this helps.

    They sent me an email, and it is the exact same information as in the first AOR from CIC, except, it was signed from Buffalo. My address also disappeared 2-3 weeks ago...
