AD is the branch coad and form number is not necessity if you have just fill it otherwise leave it empty. In my case I just filled AD code.

Hello, i just got the approval of gic from cibc bank. However there are two benificary account no and names ( one is mine and another is of a bank of Mumbai) to which account no i need to deposit the funds?
