In your receipt which Dr gave you, do you have UMI numbers or IME. If there are UMI numbers ( which is probably in your case, I think) then you Dr did not give UMI details to CIC.

You wont get RPRF right away it takes time and varies from case to case. Most of the June applicants did there medicals in December / January and still waiting for RPRF. You can pay upfront there is no harm in it. It might save you 3-4 days.

Did you select New York visa office when you applied? OR have you been in US before applying? Ottawa might have lot of applciatinos so they may be sent few application to other visa offices. I dont thing you need to worry about. Good luck
