Since you're an Indian citizen you cannot be a citizen of any other country. If you have any other type of official document issued by the Nepali government you should add the status according to that. Otherwise an explanation letter as the member above pointed out should be enough.

If you have an Indian passport you're an Indian citizen and so your status for India would be Citizen.
