Yes, QSWP applicants can do the medical exams upfront. You just need to find a clinic that will do it (some don't accept taking the exam if you don't have the requisition documentation)

Are you an outland applicant submitting the application now? If that's the case, why wouldn't you have to pay for the dependent? If you're an inland applicant, the newborn is Canadian so I don't think you have to pay for the dependent, but you have to declare the baby's very existence.

There's no answer to that question. There's people with medical exams done since February and no sign of decision yet.

That's right. Probably more than half of the 2018 applicants didn't get MR or DM yet. I don't want to be the bearer of bad news, but the best thing for the 2019 applicants now is just to forget about their applications until at least next year. For your own sanity's sake.
