Over a year, they won't request new medical until they start to touch your file.

Expired means expired, if they will finalize your application soon and your medical result expire soon, they may extend that for a couple of months for you. But if they won't process your file soon, they won't extend that for you, expired just means expired. I got the next week appointment...
