Whatever is mentioned in your evaluation have to be contained in your profile, missing anything may endanger your application. If you really don't want to report one or more of your degrees, then get a fresh evaluation from one of the many available agencies.

If you have done an MBA , then you only need to get that done and as long as IQAS or another agency says it's equivalent to Master's Degree then you are good to go. Adding you bachelor's degree is only a plus to IRCC and you won't get points to it, but it doesn't add extra effort from your side...

NOC1111 would do fine if her duties listed in her experience letters are related to those under NOC1111. Payslips and signed contracts can proof that it was a regular job regardless of the actual title

You don't need the physical card , just the number. If you been issued one , try to look around for a scanned copy of it or speak to your previous employer they must have one in file, just get the EID number
