    @PreetKaur1..... As per Legalfalcon's reply on the same ... We do need to validate the form before submitting. As per my understanding, you should immediately resend the form after validation alongwith letter of explanation.

    medical test for infant < 1 year

    Book the appointment with on-panel physician/hospital (preferably with e-medical system) and they will let you know the required documents and process. Having said that, its just a physical check up and nothing else.

    Yes fill the form as Main Applicant and add spouse and child as dependent.

    You do need to sign it ... Validate the form and it will generate barcode pages in the end of file. This barcode generated file will suffice.
