    In my opinion logically, i would put the date of last entry when submitting the form. The form ia generic, your case is special so i dont see any problem just put an honest answer

    Don't rely on GUs they are not reliable. They have system issues sometimes notifications are not sent. Your best bet is to call you loose nothing
