Here's my guess as to what happened: There is no record of your entry as a visitor in CIC systems. CBSA probably does so the CBSA report would definitely help. Do you have an entry stamp in your passport for the 2010/Nov/01 date?

Were you living in Canada between 2010/Nov/01 (date you entered as a visitor) and 2012/Jan/12 (date of permanent residence)?

I am curious how you ended up with a citizenship certificate that has an extra name if your birth certificate did not have it. Did you have the middle name on your PR card and on your foreign passport?
