Also, try to collect as much as possible documents from employer that should support your reference letter. Be honest in, what documents you are submitting.

You obtain the reference letter from employer printed on companies letter head, signed by authorized person (his contact details for future communication, if needed) and get companies official seal on the letter. The letter should clearly indicates about your CTC, mode of payment, working hours...
