There are times when CIC refuses to give PR to people if they find that they haven't been filing their taxes (thus showing lack of responsible citizenship). I know of people who have been deported for this reason. I'm assuming that's the reason.

Check out the Toronto Birth Centre. They might have options for you. But they don't offer interventions like epidural if you're into that.

Since you are in Vancouver that will probably be the case for you. I didn't get an email for DM, I just got the landing letter the same day as I got SA and AIP.
