Would you happen to know the answer to this question: 5 Use of a representative (IMM 5476) - As the Sponsor I am also filling in the forms for my parent. Should I still complete the Use of a Representative as myself? ANSWER: I am not filling this form as I am the sponsor not a representative.

I applied in October 2010. I have requested 2010 and 2013 Option C from CRA (tax dept). I hope this is correct.

Same thing for me for my parent visa application. I received a letter dated 30 July 2014 on 29 July 2014. The checklist is missing items 4 to 7. I am sending back the original form today with a letter asking for a complete checklist.
