Citizenship Application - Quebec Names

I am trying to apply for Citizenship and I am a resident of Quebec. My immigration and all my Canadian documents (drivers license and health card) are still on my Maiden Name (No plans to change it too). However, my passport is in my Married Name.

My question is for Question 4B - "Have you legally changed your name?" - Should I answer this as Yes or No?

The condition is:
If you have legally changed your name outside Canada and are residing in Canada
Then you must provide a copy of the following documents:

  • A foreign passport or other national authoritative documentation amended to reflect the new name;
  • A foreign name change document that links your previous name to your new name, such as a foreign marriage certificate (with an official translation); and
  • A document in the new name from Canadian provinces or territories (ex. driver's licence, health card, age of majority card, senior citizen’s identification card, or social service card)

Any thoughts from applicants with the same situation that are from Quebec? Thank you!

I would assume that you had to do /something/ to get your married name on your passport, no? (such as filing for a name change request, etc.) I have a friend that's in a similar boat (her RAMQ card is in maiden name, but passport is in married name.) I'll ask her what she did, but, as usual, I would err on the side of caution and say "yes" and include whatever documentation you have to support why your passport is in a different last name. It's fairly well-known that Quebec is a bit squirrely on the whole "maiden name" thing on official documents.

Hi.... thanks for taking the time to respond here. You're right, Quebec's not allowing us to change to our husbands last name. And yeah, my PR and Ramq's on maiden, passport was renewed last 2020 and had to change it to last name of my husband. But all in all, all my docs here in QC Canada are still on maiden and im okay not changing it. Just confused on how to answer the form.
