If these are the names as it appears in my passport:
Surname: DOE
Given Name: JANE
Middle Name: SMITH (mother's maiden name)

In filling up IMM1344 or anywhere in the forms that Given name is asked, should i put JANE SMITH as Given Name? since there's no box for a Middle Name anywhere in the application. Or just JANE?

Please help guys.

your "given name" is always your first and middle name. if you have a middle name but it doesn't appear on your passport don't put it on the forms. if it does appear on your passport, include it.

for example, my husband's name on his passport is Ryan Eric so that's what was filled out for given name(s). however, even though his children have middle names their passports only list their first names, so we only wrote down their first names, as it says "exactly as it appears on your passport" for the instruction for all the forms.

in your case, it sounds like you would put Jane Smith for everything.

As long as the forms match what your passport says as marlasinger advised. So if your passport says both of those names above in that order, then on the form your surname would still be Iqbal Sherif and your given name would still be Ibrahim Sherif. You would not put in a middle name as this is not what your passport says. Most immigration officers know that there are different uses of surnames and given names in different countries so I do not think you will have any problems. Blessings.
 
I was not able to include my middle name (my surname when I was not married yet) in the forms I submitted; will I be having any problems in the future? ( btw, I'm only waiting for my passport to be returned to me )
 
As long as you are not settling in Quebec, then no, you will not have any problems in the future.

I think you need to include all your given names. That includes first and middle name.
 
Hi,
My educational mark sheets are printed with the following sequence:
Last name, first name and middle name

However in the passport,
On the first page - it is First name (Given name) and Last name and another page has my father name which is basically a middle name but given name does not have a middle name with my first name.

So my concern is by which sequence name shall I opt for my ECA report that I shall not face any challenges later for the PR

where should I put the 'ahmed' in middle or in the first name?
