Question about IMM 5710 E

Hi everyone,
I am waiting for an answer to my Tr to Pr application and my visa will expire soon so I’m now applying for an Open Work Permit. On the form IMM 5710 E, in the box about Employment, if I am still currently working do I write it down in the first box? Also it asks “From” to “When” but what do I write down if I’m still working at the same company?
