I applied online from Pakistan for Visit visa on 29th October and received the Passport Submission Request on 27th Nov. I have a valid used US visa with 3 visits in the last 3 Years.

Yes the bank statements of last 6 months in the section of Financial Means. I also attached my tax returns, property papers, Salary Slips etc.
