Upfront. Unfortunately, i received a request for medical exam the same day i went for the exam. So my status was "your action is required" for more than a month.
Most likely there won't be any update on your application for a month, so i would say get it done upfront. But if you have the patience, then I would say wait for the request from IRCC and then go for your medicals. The latter takes lot less time to show on your application.
