Can you guys please let me know where to upload this form in the new Ircc portal because there is no field to upload this document? as per IRCC, the below is an updated document checklist for fully vaccinated travelers.

Proof of vaccination is only needed while traveling. You don't need to submit it as a part of visa application. Check last column.

Guess that is what you are asking.
 
I am also confused by the information on the website. IMM 5645 is list as a required form in the website but the new IRCC portal does not require you to submit one.

Does anyone know whether I need to submit a signed IMM 5645 if I use the new portal?

I don't know what you applied for; I've never seen such a table nor do they fill forms in the new IRCC Portal.

Hi. What did you do then? I am applying for my mother's visit visa. I am not able to find the place to upload the form.
 
imm5634 is for GC key applications. If you do the IRCC portal, there are questions there for family info (parents and children) included in the online application which you have to fill up.

Can anyone cofirm if imm5645 is required when applying via IRCC portal? Applying for TVV but it doest ask for it in supporting documents section
