For all intents and purposes that matter here, a letter is an affidavit - a written statement. (A more formal sense used in courts and the like is a statement sworn before an official, officer of courts, etc - the value being it's akin to perjury if untrue, but that's not relevant here).

Don't complicate things - the letter itself is fine unless something more formal is specifically requested.

You can search but free format - just a letter, addressed to no-one in particular or IRCC or to whom it may concern, sign, date.
