I am thinking what to do now go for deferment and upload new letter of next intake via web form. Because I don't think so I will get visa this month. Because my medical appointment scheduled on 25th Aug. And my classes will be starting from Sept 8.

Perhaps you could check with your university regarding the last possible date for deferment and how that would work? Many universities are trying their best to be flexible to students who can't make it in time. Maybe you could ask them for advice regarding what to do as well? There's a good chance there are many students in the same boat as you.

Yes. University is saying If you will not get visa by mid Aug it's better to defer for next intake. But I am confused should I send my deferment letter to IRCC or just travel there in next intake on fall visa application. Would this affect on my PGWP? And also I will have to apply for visa extension, how much hectic it will be then.

I'm also in a somewhat similar boat, as I'm not a study permit applicant, but a LMIA-based WP applicant.
Waiting for an update from Abu Dhabi VO since March, 2020.
WP applicants may hear back from IRCC in the last quarter of the year. I wish you get the response ASAP
