Restore status as a worker or apply for work permit for the first time or with a new employer

I am applying for a post-graduate work permit (PGWP) since I recently graduated from university. I currently hold a valid student permit but before university I went to a different institution and acquired a diploma with a different student permit and under a co-op program so I had a work permit as well.
The IMM 5710 form is asking me if I need to restore my status as a worker or apply for a work permit for the first time or with a new employer. So I'm wondering if I actually have to select the former even though my status as a worker expired about 4 years ago and I wouldn't have the same employer.

Bonus question: Should I submit documentation from both institutions in an attempt to get a total of 5 years (my passport would be valid for the entire duration) or is 3 years the maximum for PGWPs?

you are not restoring status but applying for a new work permit. You may be entitled for 3years not 5yrs

You are selecting the wrong options. If you do it correctly you should clearly see a sub-header that says apply for PGWP

You should select 'apply for a work permit for the first time or with a new employer'
Later on in the form it will ask you what type of permit you want to apply for, there you would select Post Graduation Work Permit.
