Are Dates Considered Continuous Only if the Months Overlap?

I'm probably overthinking this, but the last thing I want is a returned application over something trivial, so bear with me...

During some of the swaps between an old job and new one across my eligibility period, some have ended and started on, for example, 2018-04 and 2018-05, but during some transfers, it all happened inside the same month.

Is the former example from 04 to 05 in the next table still considered a continuous history?

Yes. As long as there are no gaps, you should be fine.

For example:
04-2018 to 05-2018 | Working in Company X
05-2018 to 07-2018 | Working in Company Y (Part Time)
06-2018 to 09-2018 | Self Employed
11-2018 to 03-2019 | Working in Company Z

In this example, rows 1,2 and 3 have correct dates (note that there are overlaps in dates). Overlaps are perfectly fine. In row 4, there is a month missing (10-2018). So, data from row 4 is considered incomplete.

Right, and if row 4 started on 10-2018, that would be fine too? It's only a problem cause of the jump from 9 to 11?
