Including failed programs along with passed ones,and error in pcc name

Hello Altruists,
I got the chance to register through the new IRCC portal (limited rollout) for study permit application. Unlike the normal GCkey login, which (to my knowledge) uses a few forms, this one directly asks about the info in different tabs.

I have previously studied Bachelor in Canada, but failed out. It is a mandatory part (already filled in a previous tab) of my education history. Then i completed my bachelor in Malaysia. So,

1. Should i include the transcripts of this failed out programs along with my completed degree certificate?

2. The study plan is supposed to be about my current program. So, should i mention anything about these failed out programs there?

3. I have seen that people usually include both SSC and HSC docs. The Education history part specifically mentions post-secondary institutes. As i am applying for PGD, should i include HSC and SSC certificates (and mention both in education history)?

4.My police clearance from Malaysia, where i did my bachelor, has mistakenly excluded a single letter from my Given name. As correcting it may take a while, should i include an affidavit with it, or just an explanation letter with it is fine (as the passport number and everything else is fine) ? or should i just wait and get it corrected (may take a week or more)?

1. You don't need to include failed one but completed degree. You need to overcome why you failed your course at first place as that is considered as you did not meet conditions of your study permit. be honest and explain what happens, and that you left without any further violations if no
2. You should, they have your records in case if your previous School did annual submission as part off the conditions of the eligible DLI.
3. I don't think you need to, your recent higher qualification will do
4. Submit that because of time and affidavit
