I should go to one of the physicians mentioned in this list to get a medical exam: https://secure.cic.gc.ca/pp-md/pp-list.aspx? Do they ask for my application number or something or I just tell them I need it for PR?

You just go an ask for an upfront medical exam for immigration and they'll tell you what to do next.

I am working as part time associate in Walmart. I have all the other requirements related to IELTS, education, work permit etc. Considering my employment in Walmart as part time Ecommerce associate, am I eligible to apply? I have met the other conditions of this stream including ielts, work permit etc
