Query :Use of Any Other Name

Hi Seniors,

This regarding Citizenship Application form CIT 0002 (10-2020) :::Q5) Have you used any other name:
My query is based on assumption that : "Family Name" , "Last Name" and "Surname".............are one and the same

My name as per:
Given Name (First name)Family Name (Last Name)
PR CARD /CANADIAN IDs/CURRENT INDIAN PASSPORTABC +PQRXYZ
EXPIRED INDIAN PASSPORTABC+PQRXYZ (added later as an endorsement)
Prior IDs in INDIAABC+ PQR---------

I even mentioned the same in my PR Application and had submitted an affidavit mentioning: ABC+PQR IS THE SAME PERSON as ABC +PQR+ XYZ

Do I need to mention this fact in the answer to this question? Yes or No

If yes:
-What reason: "Alias/Nickname" or “Name at Birth”
-The form does allow the column for Family Name to be left blank…how should I fill it?
-Do I have to submit any proof or just mentioning is fine ?

You can always call CIC and ask, they are usually pretty helpful. I’m not an authority and this is just my 2 cents but I was told ones that those are questions about any official names ever used on official documents. Being in your shoes I would repeat what you did for your PR application. As they always say over the phone, it’s always better to included more information than not enough.

Is “ABC+PQR” your first and middle name? Are you saying that you didn’t have a last name on your earlier documents in India?

"ABC" is first name
"PQR” is Middle name

I didn't have a last name "XYZ" in majority of my earlier documents in India?

I would include that. It sounds like it would be your name at birth. If you haven’t had a last name at some point in your life you can probably keep it blank if the form allows it. There is a line in the official CIC guide for question 5 saying: “
  • If you do not have a given name, leave this field blank. Do not enter “*”, “Not applicable” or “NA”.”
It’s about given names so I’m not sure if it would make a difference if you left the “last name” column blank. I would also include that document you mentioned plus a short explanation letter. They might take a look at you PR application so I think it would ba a good thing if mentioned names match.
Again, this is just my guess, I am NOT 100% sure that I’m correct, just speedballing here.
I would strongly suggest to call CIC and just ask them, they will most likely answer your question.

Use the same name that was used for your PR application. They already have that in their record. Just my 2 cents.

Try giving a blank by hitting space in the box and see if it works.
