I am not a doctor AND I cann't give medical advice.
That said, here is what usually happens.

They find some abnormality in your pulse rate OR blood pressure OR they hear some murmur in your heart. What hospital will do during that visit of yours is that they will ask one or more test. That is usually ECG. IF there was an issue in ECG test -- which is rather very obvious when they take it as the machine itself tells it, they will do even more test, usually a 2-D echo. IF there was high blood pressure a kidney function test is also done (it usually means one more blood test.)

Now assuming they did only ECG, likely, ECG was not bad. Otherwise, hospital at that point would have asked for the 2-D echo / treadmill test OR would have asked you to book an appointment for it then and there. This is because it makes clinic/hospital some money AND it speeds up things. If you send all the reports to IRCC, things go faster. Its benefitial for both clinic and for you too so it is usually done this way.

There is also a chance that IRCC may insist on a "furtherance" and will make you take a 2-D echo / TMT even if clinic/cardiologist says okay to you.

Kidney function test takes a lab report.

Now if further issue shows up in 2D echo or TMT, it will lead to a consultation with a cardiologist. Assuming you have no history of heart troubles, they will go over the family history of heart issues.

Now what IF there is something really wrong. Well, that depends upon what is wrong. Cardiology is a whole discipline in itself and beyond this, a doctor can only tell.

Judging by your disposition in this thread, most likely it is not. You were likely nervous during your medical checkup (and possibly did not sleep well the last night). It caused your systolic blood pressure to shoot up to 160-180 range. This coupled with doctor seeing a fast heart rate of 95+ (again due to nervousness) caused them to do a ECG test. Recommendation from IRCC (AFAIK) for > 140 systolic is to do a heart evaluation by a cardiologist. Cardiologist will at minimum do an ECG. Which was likely done. Another recommendation is to do a kidney function test. You have not mentioned it but I guess they did it. Using the same blood drawn OR may be one additional blood sample taken after all your other tests were done.

Again, above is my common sense guess. I don't know anything about your health or your medical history nor have medical training. Rest, your doctors will tell you.

Unless your doctor says so, its not. The ECG can detect arrhythmia and other issues in your heart's electric function. If such were there, it would have shown up and it would have been written in the report somewhere. Medical term for fast heart rate is "Tachycardia". Its just an observation not necessary a confirmation of disease. It can happen because of many reason mostly but most common reasons are stress and exercise. At 22, your maximal heart rate is 220-22 = 198. Unless you are fit and have been exercising, your heart rate should be around 72 when resting. That said, when stressed it can go up.

It is not impossible to have 138 bpm when resting and under a lot of stress. Given you are so much worried about your medical test, I do not even know how much worried you might have been when you were in hospital and being told out of blue that an ECG needs to be done. I am guessing you never had an ECG done? right? A bunch of electrode attached all over you body can be scary as hell. If you were scared your blood pressure and heart rate would shoot up like hell.

I will suggest you to take it to a doctor, ask them about it. Their advice only can put your mind to rest.
