Hi, I have a question about Part B on the form to apply for OHIP while having a post graduate work permit and full time job.

It says "When did you take up permanent residence in Ontario"? Does that mean when I first came to Ontario from my home country to study? Because I dont have my PR yet, I have my post graduate work permit after finishing my studies. If its regarding PR, should I just leave it blank since I dont have that yet?

Thanks for any help

I left that blank because technically we are considered temporary residents.

Ok great, thats what I thought of doing as well. So you did that and everything went through ok? Was the job letter required to be signed and dated in pen?

Thanks for the help

My husband was the one that applied because he was working full-time at that time. His job letter needed to state the date of employment, that the position is full-time and permanent, and that the company intends to keep him employed beyond 6 months or indefinitely. It had to be signed and dated by HR as well. We received our temporary(paper) health card the same day. The real card came about 2 weeks later in the mail. Hope this helps.
