you would either get a medical extension letter or an updated "medical pass" date on your GCkey account from my understanding

OK. good to know that there's no theory or basis on which they extend medicals then. it's all depends on visa officer on the file I guess.
They have not , I have GCMS notes twice they have not started working on the eligibility . Last time they ask me document was the use of rep form for myself which is bizarre. Other than that no additional document requested just application received status. What is your date of lock in ? Have you received AOR?

Mine was received on June 12, 2019. Ecas shows in process but only states application was received on June 12. Very frustrating!
No they havent yet. I called IRCC but nothing they can do. What about you?

I remember reading about this somewhere but I received the pre-arrival letter today in the myCIC portal:

Good that London VO moving fwd your application. keep eyes on your ECAS there is chance that your file status update soon to DM, as per tracker from London VO few applicant got DM within 2-4 week after pre arrival service letter but in your case your medical soon expired.
Keep eyes on your Ecas for DM & myCIC to check your medical passed date . if they make decision before medical expired VO give you extension for medical and will update Ecas to DM.
