Ideally, your paystubs should also be attested by some company official. If you are not able to get that, you can still use pdf files in support to your reference letter.
If company address is not available on reference letter, include your appointment letter or some official letter sent to you containing company address or screenshot of company website containing address and business card( if available) in the same file as that of reference letter. No need to include that in LOE.
