Hi All,

1. I'm planning to send photo copy of Driver's License and Health Card for identification docs. Question is should i send both sides of them?
2. Also for my kid, i'm gonna send passport copy and health card? Is it sufficient? (health card does not have a photo..)

1. Yes, send in both sides.

2. If I remember correctly, you need to have a photo, signature & D.O.B on all IDs you submit. This information is on the Document Checklist so have a look there. Worst case, you can request a Photo ID - Ontario issues one for sure, but you would have to check in other provinces.

EDIT:
I was correct; here is the excerpt from the Document Checklist:
"The document must have your name, photo and date of birth on it. If there is information on both sides of your personal identification documents, provide a photocopy of both sides. See Step 3 of the instruction guide. You cannot use your permanent resident card as identification."

You need two IDs and one needs to have a picture see below.

Photocopy of two (2) pieces of personal identification.
Both pieces of identification should show the minor’s name and date of birth, one of which must have a photo on it.
Examples include:
  • school record;
  • health insurance card;
  • a passport page containing their photo and personal details;
  • a hospital or immunization record may be provided.
  • Note: You cannot use the minor’s permanent resident card as identification.
  • Note: If there is information on both sides of the identification document, photocopy both sides.
